Helicobacter pylori

Helicobacter pylori (H. pylori) is a gram-negative spiral bacterium capable of transforming into a coccoid form, which enhances the survival of bacteria in the stomach when exposed to aggressive factors. The spiral shape and the presence of flagella provide mobility of the bacteria, the coccoid form is capable of colonizing the gastric mucosa. H. pylori can form biofilms, which reduces its sensitivity to antibacterial drugs. It has been shown that H. pylori has both a direct damaging effect on the gastric and duodenal mucosa due to the synthesis of an enzyme complex (urease, protease, phospholipase) and cytotoxins (VacA, CagA), and indirect damage to the mucosa due to the release of interleukins, lysosomal enzymes, tumor necrosis factor accompanied by inflammation. H. pylori can be the cause of gastrointestinal diseases such as:

  • chronic gastritis;
  • peptic ulcer of the stomach and/or duodenum;
  • gastric cancer;
  • MALT lymphoma.

Timely detection and treatment of Helicobacter infection can prevent serious complications such as peptic ulcer disease and gastric cancer. Screening for Helicobacter infection is recommended for patients with symptoms of dyspepsia, chronic gastritis, peptic ulcer disease and a family history of gastric cancer, as well as to those with idiopathic thrombocytopenic purpura and iron deficiency anemia of unclear genesis, as the infection may be associated with these conditions.

Indications for testing

  • Chronic gastritis
  • Peptic ulcer of the stomach and/or duodenum
  • Dyspepsia
  • Family history of gastric cancer
  • Iron deficiency anemia
  • Vitamin B12 deficiency
  • Idiopathic thrombocytopenic purpura
  • Planning long-term therapy with proton pump inhibitors and/or NSAIDs
