HBV

Hepatitis B virus (HBV) is a DNA-containing virus belonging to the family Hepadnaviridae. HBV is one of the main pathogens causing acute and chronic liver disease, including cirrhosis and hepatocellular carcinoma.

Hepatitis B virus is transmitted by parenteral, vertical, and sexual routes. Parenteral transmission involves contact with infected blood and bodily fluids, which can occur through blood transfusion, use of non-sterile needles and instruments, and contact with damaged skin and mucous membranes. Sexual transmission occurs through contact with infected bodily fluids such as ejaculate or vaginal secretions. Vertical transmission occurs from mother to child during childbirth.

The main infection mechanism is the virus entry into hepatocytes, where it integrates into the host cell genome, which can lead to chronicization of the inflammatory process and the development of complications.

The acute form of hepatitis B is usually accompanied by activation of T-cells, production of cytokines and neutralizing antibodies, which contributes to virus elimination. In 10% of cases, hepatitis progresses from acute to its chronic form.

Patients with suspected acute hepatitis B infection are advised to do a PCR-based blood test for HBV DNA as a criterion for early diagnosis as well as to establish the viral load. HBV DNA becomes detectable in blood on average one month after infection and 10-20 days before the appearance of HBsAg. In addition, qualitative testing for HBV DNA is performed to assess virological response to treatment with nucleoside/nucleotide analogs.
